Sonic Hedgehog (SHH)
De verwachtingen zijn hooggespannen rond het kleine Amerikaanse farmaceutische bedrijf Curis, dat op dit moment aan het experimenteren is met een compleet nieuwe behandelingsmethode voor onder andere haaruitval. Bij deze nieuwe behandelingsmethode draait het allemaal om een recentelijk ontdekte eiwittengroep, genaamd Sonic Hedgehog.
Dit eiwit, dat samen met de Desert Hedgehog en de Indian Hedgehog tot de Hedgehog-familie behoort, blijkt zeer bijzondere eigenschappen te hebben. Een studie uit 2001 heeft aangetoond dat de Hedgehog-familie een ingewikkeld netwerk van eiwitten is dat cellen gebruiken om te communiceren, en dat het een belangrijke rol speelt bij de groei en ontwikkeling van nagenoeg ieder essentieel orgaan. Het communicatiemechanisme van de Hedgehog-eiwitten is vooral in embryo's erg actief, waar het verantwoordelijk is voor de formatie van bepaalde organen.
Wetenschappers hebben ontdekt dat de signalen van de Hedgehog-eiwitten kunnen dienen als een soort moleculaire schakelaar. Aan de ene kant kunnen de signalen de vermenigvuldiging van cellen stimuleren. Dit kan van pas kan komen bij de behandeling van haaruitval, doordat de geminiaturiseerde haarzakjes hun oude vorm weer kunnen aannemen als de celdeling weer op het oude niveau geraakt. Aan de andere kant kunnen de signalen van de Hedgehog-eiwitten de celdeling ook juist stopzetten. Dit maakt de eiwitten bijzonder interessant voor behandeling van bijvoorbeeld kanker, hersenaandoeningen en vele andere ziekten.
Curis onderzocht de Hedgehog-eiwitten uitgebreid (met name de meest potentiële, de Sonic Hedgehog) en vroeg uiteindelijk in 2003 en 2004 een aantal patenten aan op moleculen die in staat zijn om de signalen van de eiwitten te sturen. Curis heeft op dit moment patenten voor de behandeling met Sonic Hedgehog van onder andere kanker, nieraandoeningen en haarverlies. Vanwege het gebrek aan financiële middelen was het kleine Curis gedwongen in zee te gaan met farmaceutische partners als Wyeth Pharmaceuticals en Genentech Inc. Op dit moment helpen deze bedrijven mee met het onderzoeken van de mogelijkheden van deze bijzondere eiwittenfamilie. Om de kostbare studies te financieren, is Curis van plan het medicijn eerst als haargroeimiddel op de markt te brengen. Van de inkomsten van de verkoop hiervan kunnen dan de overige onderzoeken betaald worden, zo hoopt het bedrijf.
Dat is de reden dat de onderzoeken naar het SHH-eiwit als behandeling van haarverlies op volle toeren bezig zijn. Begin 2005 werd de zogenaamde preclinical trial afgerond, die uitwees dat het middel uitstekend werkt om op een veilige wijze de haargroei bij muizen te stimuleren. Algemeen wordt verwacht dat soortgelijke resultaten ook bij mensen te verwachten zijn. Volgens de laatste berichten is Curis van plan om de eerste reeks experimenten bij mensen al eind dit jaar uit te voeren.
Behandeling met SHH verschilt vooral van huidige behandelmethodes tegen haarverlies in het feit dat het zich richt op herstel van de aangetaste haarzakjes. De huidige generatie haargroeimiddelen richt zich alleen op het wegnemen van de oorzaken van haarverlies. In theorie zou behandeling door middel van SHH-signalen daarom niet alleen kunnen helpen tegen alopecia androgenetica, maar ook tegen alopecia areata en andere vormen van haaruitval.

A gene called sonic hedgehog is also making headway. A company called Curis already is trying to tame sonic hedgehog for hair growth.

"Sonic hedgehog can convert resting hair into growing hair," Cotsarelis says. "We really don't know it's full role, but if it controls follicle size and growth, it could be something a treatment might be based around."

Sonic hedgehog is just one of several key genes scientists are tinkering with in labs.

If this all sounds futuristic, it is. But there are good reasons this kind of technology will move forward. Today, Americans spend $800 million on hair restoration surgery. And they'd spend a lot more if the surgery got faster and better.

"If it can be done, it will be done," Barrows says.

CAMBRIDGE, MA, October 28, 2003 - Curis, Inc. (NASDAQ: CRIS) today announced that it has been issued a U.S. patent claiming a method for promoting hair growth through the use of agonists (activators) of the Hedgehog signaling pathway. U.S. patent 6,639,051, issued today, is entitled "Regulation of Epithelial Tissue by Hedgehog-like Polypeptides and Formulations and Uses Related Thereto."

The Hedgehog signaling pathway is one of the major regulatory mechanisms responsible for the development and maintenance of many tissues and biological systems in the body. One of those biological systems involves the hair follicles and the stimulation and maintenance of hair growth. Curis is using the Hedgehog signaling pathway as a technology platform to develop drug candidates to treat several different kinds of diseases, including hair growth disorders.

Usually hair loss is not caused by a disease, but is genetic. Inherited hair loss or "pattern baldness" can affect both men and women but is more common in men. According to the National Insitutes of Health, about 25% of men begin to bald by the time they are 30 years old, and about two-thirds are either bald or have a balding pattern by age 60. Hair loss can also occur as a result of cytotoxic drugs and procedures, such as chemotherapy.

Dr. Lee Rubin, Curis' Chief Scientific Officer, said, "The Hedgehog signaling pathway is normally activated by the interaction of Hedgehog proteins with their receptors. One of Curis' most important scientific advances of the last several years has been the discovery and development of small molecule Hedgehog pathway activators that can take the place of these proteins. These compounds, called agonists, are traditional drug-like synthetic molecules, some of which are more active than the Hedgehog proteins in terms of their ability to activate the Hedgehog signaling pathway."

Daniel Passeri, Curis' President and Chief Executive Officer, said, "On September 2 of this year, Curis was issued a U.S. patent that covered pharmaceutical compositions of the Hedgehog agonists. The combination of that previously issued composition patent and this new method patent gives us strong intellectual property protection for the hair growth program that is under development at Curis."

Nu, twee jaar verder, is er nieuwe info beschikbaar over dit toekomstige haargroeimiddel! Het werkt via een eiwittencomplex genaamd Hedgehog (Engels voor egel ) en het is in ontwikkeling bij Curis. Ik heb er een goed artikel over gevonden op BusinessWeek Online:

Startup Curis is developing a class of drugs that could halt hair loss -- and treat cancer

If you're fighting a receding hair line, the options are probably all too familiar. There are comb-overs, hair potions, and more drastic transplants. Then there's Propecia, which may actually work -- if you can get past the warnings about sexual dysfunction. But with such treatments, the result may be little more than peach fuzz, and the hairs fall out when you stop using the drugs, says David H. McDaniel, director of the Institute for Anti-Aging Research in Virginia Beach, Va.

It's no wonder combined U.S. sales of the leading branded hair-growth tonics and drugs amount to less than $1 billion a year, despite the fact that about 40 million men in the U.S. alone are losing their hair. "People thought they'd go from Kojak to Chia Pet," says McDaniel. "But the results have been underwhelming."

A tiny biotech company named Curis Inc. (CRIS ) believes it has a promising alternative. It's an experimental drug based on a cellular mechanism with a bristly moniker: the Hedgehog signaling pathway. Named after the furry-looking mutant fruit flies in which it was first discovered, the pathway is a complex network of proteins that cells use to communicate. Scientists have discovered that these specific cellular signals can also serve as a molecular switch. Flip it in one direction, and you can wake up sleeping hair follicles. Tweak it in another way, and you might conceivably halt tumor growth in certain types of cancers, temper neuron destruction in the brain, and alter a variety of other disease progressions.

So far, the results have only been observed in animal experiments -- and everyone agrees that lab critters rarely respond exactly like humans. Still, the data are so promising that some of the world's top biotech and pharma companies have cut deals with Curis. In April, Genentech Inc. (DNA ), which has licensing agreements with Curis, said it hopes to start human trials of a skin-cancer drug this year. Curis is also working with Wyeth Pharmaceuticals (WYE ) on a stroke treatment.

HEALTHY SIGNALS
These two partners alone could shell out more than $300 million in licensing payments over the next five or six years, providing a much-needed infusion for Curis, which has been unprofitable ever since it was born out of the merger of three cash-starved biotechs in 2000. And CEO Daniel R. Passeri says that by the end of this year he expects to sign up a partner to help with human clinical trials for a Hedgehog-related treatment for baldness.

Like the mammal, the Hedgehog signaling pathway is especially frisky when it's young. In embryos, this biochemical chain reaction operates at full force, controlling the formation of certain organs. In adults, Hedgehog is less active, but it kicks back into high gear periodically to help the body recover from injuries. That power intrigues Lee L. Rubin, Curis' chief scientific officer. "The idea is that maybe in adults you can reactivate the pathway to regenerate tissues," he explains.

Hedgehog is one of many cell-signaling pathways that scientists are scrutinizing. And Curis hopes it will work wonders on thinning pates of both males and females. When you see a lustrous head of hair, chances are it's at least partly due to healthy Hedgehog signaling, which prompts the body's multitalented stem cells to proliferate around follicles, initiating hair growth. By activating Hedgehog, Curis' drug targets hair growth at its root, so to speak. That could make the drug more effective than current treatments, which mainly aim to increase the size of shrunken hair follicles or to fight certain hormones involved in hair loss.

Curis' decision to farm out some of its hair-loss research costs -- and hence share the potential payoff -- was painful. The company had planned to retain 100% rights to the baldness market, hoping to pour proceeds from any gold rush back into research on life-saving drugs. But after several companies' painkillers got pulled from the market because of safety concerns, Passeri came to a sobering realization. "Any quality-of-life drug is going to be looked at very carefully" by the U.S. Food & Drug Administration, he says. "The FDA is likely to require big safety studies -- broader than we can subsidize ourselves." Having lost $13.9 million on just $5 million in sales in 2004, Curis now sees partnerships as its most viable route.

PATCHED AND SMOOTHENED
Sharing the baldness market will help Curis conserve the cash it needs to foster its other promising drugs. Its scientists are especially optimistic about their partnership with Genentech to develop a topical treatment for basal cell carcinoma. About 1 million new cases of this form of skin cancer are diagnosed yearly, making it the most common human cancer. Surgery is effective, but it can leave unsightly scars. When a topical drug co-developed by Curis and Genentech was applied directly to samples of cancerous animal skin, the tumors disappeared and the surrounding healthy skin remained intact.

The key to the cancer treatment is not to turn the Hedgehog signaling pathway on, as Curis is doing for hair loss, but rather to turn it off. In basal cell carcinoma, the pathway goes haywire, upsetting a whole menagerie of quaintly named proteins. Normally, the Hedgehog gene makes a protein that binds to another protein called Patched, found on the outer surface of cells. Patched is supposed to repress a cancer-causing protein called Smoothened. But in basal cell carci- noma, Patched malfunctions, Smoothened runs wild, and tumors form and grow.

Curis scientists figured out how to block the abnormal Hedgehog signal with a substance that has the same effect as a chemical originally discovered in the 1950s by Idaho sheepherders. When their sheep ate corn lilies containing the chemical, their lambs were born with just one eye. Scientists later discovered that the substance, which they dubbed cyclopamine, was causing defects by interrupting the Hedgehog signal. Now they believe they can harness the chemical's disruptive power and direct it straight at tumors, potentially yielding entirely new ways of treating many different cancers. "Over the last two years, Hedgehog has been implicated in pancreatic, prostate, and some forms of lung cancer," says Fred de Sauvage, senior director of molecular biology at Genentech, which plans to work with Curis to find other potential uses of Hedgehog inhibitors.

Hedgehog's multifaceted role in the body might also be its undoing. Because the pathway seems useful in some diseases when it's shut off and in others when it's turned on, forcing it in one direction might bring on dangerous side effects.

Wyeth is looking into that prospect as it works with Curis on a drug designed to activate the Hedgehog pathway in the brain after stroke. The researchers believe that Hedgehog will decrease damage, and they hope it will mobilize stem cells to make new neurons. But when they look at the growing body of research that shows Hedgehog playing a role in some cancers, they wonder about turning the pathway on after stroke. "We have to make sure we're not causing tumors," says Menelas N. Pangalos, vice-president of neuroscience discovery at Wyeth. Curis' Rubin says he believes the risk will be tempered by the fact that the stroke drug would only be given to patients once. But no one will know for certain if any of Curis' therapies are safe until they are tested in humans.

That uncertainty may explain why investors are keeping their distance. Curis' stock has dropped from $5 a share in January to $3.40. That frustrates Passeri. And he's sometimes embarrassed by the notion that baldness could be Curis' ticket to the biotech big leagues. "Scientifically, it's not what we're most excited by," says Passeri -- who himself is blessed with naturally thick hair. "But from a business standpoint, it has huge potential." If all the bald pates in the world help Curis fund cancer research, the potential beneficiaries will surely outnumber all the sheep, fruit flies, mice, and humans that have contributed to the Hedgehog discovery.

Dit klinkt toch wel echt zeer revolutionair. Een compleet andere benadering van het probleem dan de medicijnen tot nu toe hebben gedaan.

Als ik het goed begrijp is dit een middel dat zich richt op het heractiveren van de eiwittengroep genaamd Hedgehog. Deze groep speelt een grote rol in de ontwikkeling van embryo's en is tot wonderen in staat: het kan weefsel supersnel regenereren.

Dit klinkt echt als een middel dat nog zwaar in de kinderschoenen staat en dat nog tientallen jaren onderzoek nodig heeft voordat het toepasbaar wordt. En gezien de gigantische mogelijkheden van een dergelijk medicijn bij honderden aandoeningen en ziektes, zou je zeggen dat er niet aan AGA wordt gedacht.

MAAR.... verrassend genoeg staat op de site van Curis dat Hedgehog (Hh) NU al getest wordt tegen AGA en dat de pre-clinical trial al in het laatste stadium is! Dat wil zeggen dat de Phase 1 ieder moment kan beginnen: ofwel het testen van het middel op mensen!!!!

Dit is toch fantastisch nieuws!! EINDELIJK weer eens een compleet nieuw middel met een compleet nieuwe aanpak.

Na minox, fina en duta hebben we een nieuw fenomeen waar we onze hoop op kunnen vestigen: Hedgehog!

Ja het is goed nieuws.

Er is dus onlangs een preclinical studie vrijgegeven met de resultaten op dieren:

In the study, the small molecule Hedgehog agonist, when topically formulated and applied to the skin, was able to transition the hair follicles from the resting phase of the hair cycle to the growth phase, resulting in pronounced stimulation of hair growth. The authors of the study conclude that the topical application of a Hedgehog agonist may be effective in treating hair disorders, such as male pattern baldness and female pattern hair loss.

Daniel Passeri, President and Chief Executive Officer of Curis, said, "We are very pleased with these results and with the progress that has been made in our hair growth program. We are also evaluating the requirements for clinical development in hair growth indications in order to decide on an appropriate strategy for moving this program forward."

Ik heb het bedrijf gemaild met de vraag of deze resultaten volledig in te zien zijn.

Het middel doet me een beetje denken aan PSI (OSH101), maar dan veel geavanceerder.

In het artikel dat ik hierboven aanhaal staat ook dat ze staan te popelen om het middel te gaan toepassen op serieuze aandoeningen, maar dat ze het eerst als haargroeimiddel willen ontwikkelen om zo geld binnen te krijgen voor de verdere ontwikkeling

Dit is trouwens ook te verenigen met die bevinding dat LED's mogelijkerwijs hergroei kunnen bewerkstelligen. Omdat LED-therapie ook zorgt voor celgeneratie.

- 5 tot 10 jaar als alles zeer snel en goed verloopt.
- 10 tot 15 jaar als er wat tegenslagen zijn.
- helemaal nooit als de FDA het afkeurt (bijv. vanwege bijwerkingen etc.) of als het geld op is o.i.d.
